Are you passionate about providing high quality care in an integrated team setting?  Are you comfortable with electronic health records and driven to improve the health of the community you serve?  Are you bilingual in Spanish?  Do you have a commitment to the Lyon-Martin Health Services mission? If so, HealthRIGHT 360 is currently looking to fill a Physician position opening at our Lyon-Martin Health Services program in San Francisco, CA!  The ideal candidate is comfortable with and has experience in trans health, and is familiar with treating issues like addiction, the needs of individuals experiencing homelessness, HIV, and Hepatitis C.  The ideal candidate is also comfortable treating chronic disease and mild to moderate mental illness.  In addition to advocacy and care for an under-served population, you will gain increased knowledge of addiction treatment across a continuum of care.  You will also learn about integrated care beyond the clinic to include substance use disorder and mental illness treatment and social determinates of health.  Sound interesting?  Below you will find a more detailed description of some of the duties and requirements for this position.

Queer Health Center seeks Primary Care Physician! Lyon-Martin is a non-profit community clinic in San Francisco that provides primary care and mental health services in an integrated model to the LGBTQ community.  We have a diverse patient population and a fun and friendly work environment.  Our caring and committed staff work as a team to fulfill our mission so that all of our patients have access to comprehensive, quality health care.  We are looking for a dynamic physician to join our team.  The ideal candidate understands that offering patients a professional setting where they are treated with dignity, respect and compassion isn’t just lip service; it is the way and reason that we run our organization.

Key Responsibilities

  • Provides preventive oriented ambulatory primary care as part of an integrated health care team in a Patient Centered Health Home delivery model.
  • Provides primary care to those with chronic disease, addiction, mental illness.
  • Participates in the clinic performance improvement program, which may include monthly QI team activities, peer review, etc.
  • Works effectively in an integrated health care team including active scheduled management, team huddles, chart preparation, working collaboratively with team members, etc. and can provide support to and work closely with behavioral health for consultation support.
  • Participates in weekly Integration meetings.

Education and Knowledge, Skills and Abilities

Required:

  • M.D. or D. O. degree from an accredited United States medical school
  • Successful completion of internship and residency in primary care specialty with experience managing patients with substance use disorder/addiction
  • Proficiency in a language in addition to English, specifically Spanish
  • Board Certified in family medicine or internal medicine
  • Valid and active California medical license, without any limitations on license to practice in specialty
  • Eligible to participate in the CMS Medicaid/Medicare reimbursement programs.
  • Valid and active DEA license with X waiver to prescribe buprenorphine; if does not have X waiver must obtain within 6 months of hire.
  • Current BLS/ACLS certificate or recertification within 2 months of hire
Compensation: $200,000.00
