Care Coordinator - NCSH Outpatient

Job Overview

North County Serenity House Outpatient, provides treatment services for women seeking recovery from substance use disorders. We offer gender responsive, trauma informed, person-centered services and evidence based interventions. For clients with co-occurring mental illness, we provide integrated substance use and mental health services which treat both conditions as primary. Care Coordinators are responsible for assessing client and children needs and strengths in support of family health and recovery. Through case management in individual and group settings, delivering curricula in accordance with program specific requirements, Care Coordinators assist clients with children in navigating systems of care while providing a supportive treatment environment.



Key Responsibilities

Individual Treatment Responsibilities: Facilitates individual case management sessions with each caseload client who is involved with Child Welfare Services. Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status. Facilitates monthly case conferences for clients who are involved with Child Welfare Services which include all parties involved in client’s case. Attends Team Decision Making Meetings when invited by Child Welfare Services and maintains regular contact with CWS workers to allow for advocacy for each client.

Treatment Setting Responsibilities: Reports to other referring agencies weekly. Performs crisis intervention and communicates with treatment team as unforeseen situations arise. Documents client updates, incidents and changes in visitation. Attends required trainings and meetings. Plans and facilitates client celebrations and special events. May work weekends and holidays as needed.  

Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client. Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements and other duties as assigned.



Education and Knowledge, Skills and Abilities



- Drug and Alcohol Registration recognized by DHCS

- High School diploma or equivalent

- First Aid Certified within 30 days of employment

- CPR Certified within 30 days of employment

- A valid California driver’s license



- Drug and Alcohol Certification recognized by DHCS

- Bachelor’s Degree in related field  


Background Clearance

- Ability to obtain and maintain background clearance with successful discharge from probation or parole, and absence of drug/alcohol related arrests or convictions, for at least 3 years

- Must not be on active parole or probation  



 - Culturally competent and able to work with a diverse population

- Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications

- Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency

- Knowledge of co-occurring disorders and trauma informed treatment

Knowledge of clinical documentation (treatment plans, progress notes etc.)

- Experience working with criminal justice population

- Bilingual
