Case Manager

If so, HealthRIGHT 360 is looking to fill a temporary full time Case Manager position in San Bruno, CA! The Roads to Recovery Case Manager is responsible for a caseload of men in a gender responsive treatment program at the San Francisco County Jail. In conjunction with staff team, develops a treatment plan focused on recovery from substance abuse and other issues that could prevent clients leading a clean, sober and crime free life (i.e. life skills, health/mental health problems, educational/vocational challenges, etc.).  Facilitates groups, classes and one to one counseling sessions. Utilizes both internal and community resources available for these clients.  Goal is to transfer these clients to Aftercare or another treatment program after release from jail.

Key Responsibilities

  • Facilitates individual counseling sessions with each caseload client addressing in-custody and aftercare needs
  • Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals
  • Facilitates group counseling sessions and community meetings and teaches classes on various treatment-related topics
  • Performs crisis intervention and communicates with treatment team as unforeseen situations arise
  • Attends required trainings and meetings
  • Plans and facilitates client celebrations and special events
  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client
  • Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner
  • Follows program and Sheriff’s Department norms, demonstrating honesty and a willingness to listen to clients’ issues, and taking responsibility for own actions
  • Works in collaboration with Sheriff’s Department by attending meetings and trainings, and upholding the norms, policies, and procedures of Sheriff’s Department

Education and Knowledge, Skills and Abilities

  • Drug and Alcohol Certification recognized by DHCS or Bachelor’s Degree in related field and Registration with Drug and Alcohol Certification recognized by DHCS
  • 1 year experience in in-custody setting
  • 2 years of college studying psychology, sociology criminal justice, women’s studies, ethnic studies (1 year of additional in-custody experience may be substituted for 1 year of college)
  • Must not be on active parole or probation
  • Maintain security clearance from Sheriff’s Department
  • Culturally competent and able to work with a diverse population
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and Internet applications
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data
  • Professionalism, punctuality, flexibility and reliability are imperative
  • Knowledge of co-occurring disorders and trauma informed treatment
  • Knowledge of Clinical documentation (treatment plans, progress notes etc.)
