North County Serenity House Outpatient Program provides a gender responsive, trauma informed approach to assist our participants in recovery, relationships and relapse prevention. Our comprehensive program includes therapy, psycho education classes, counseling groups, advocacy, onsite daycare, homeless supportive services and exit planning. We believe in individualizing participants’ treatment based on their need and what works best for them. Our hope for every woman is that they are able to improve their quality of life, establish a support system and develop healthy coping skills.


Key Responsibilities

The primary responsibilities of the Case Manager/Driver include providing direct participant care in a manner that exemplifies HealthRIGHT 360’s philosophy, goals, policies, mission and vision through the monitoring participant behaviors, maintaining regular contact with participants, participants' families, treatment providers, employers, and various other stakeholders, and promoting and facilitating pro-social activities and behaviors with participants experiencing addiction and potential co-occurring disorders. The Case Manager/Driver position reports directly to the Program Manager. Case Manager is responsible for providing guidance to participants and their children to meet their health and human service needs. 

Case management is a collaborative process with the treatment team and the participant which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual participants’ needs. Facilitates participants Life Skills and Vocational Readiness groups, i.e., Job Placement, Job Retention, Employment Barriers, Computer Skills, Motivation, Money Management, etc. Provides specialized case management services. Conducts random drug and alcohol screenings of participants. Maintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) 42 CFR, agency, and funder standards. Completes accurate and timely data entry into the EHR system as applicable and other required internal and external databases. 

Education and Knowledge, Skills and Abilities

Education and Experience Required:

  • Drug and Alcohol registration recognized by Department of Health Care Services (DHCS).
  • Minimum of 1-year Case Management or related experience.
  • High School diploma or equivalent.
  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.
  • A valid California driver’s license and automobile insurance.


  • Valid Substance Abuse Counselor Certification from an accredited organization.
  • Experience working with criminal justice population.
  • Bilingual English/Spanish.

Compensation: $38,000.00-$41,000.00
