The primary responsibilities of the Case Manager/Driver include providing direct participant care in a manner that exemplifies HealthRIGHT 360’s philosophy, goals, policies, mission and vision through the monitoring participant behaviors, maintaining regular contact with participants, participants' families, treatment providers, employers, and various other stakeholders, and promoting and facilitating pro-social activities and behaviors with participants experiencing addiction and potential co-occurring disorders. The Case Manager/Driver position reports directly to the Program Manager. Case Manager is responsible for providing guidance to participants and their children to meet their health and human service needs.
Case management is a collaborative process with the treatment team and the participant which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual participants’ needs. Facilitates participants Life Skills and Vocational Readiness groups, i.e., Job Placement, Job Retention, Employment Barriers, Computer Skills, Motivation, Money Management, etc. Provides specialized case management services. Conducts random drug and alcohol screenings of participants. Maintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) 42 CFR, agency, and funder standards. Completes accurate and timely data entry into the EHR system as applicable and other required internal and external databases.
