Case Manager - Licensed Vocational Nurse

Job Overview

Prototypes intensive outpatient program consists of Full Service Partnership and Field Capable Clinical Services Adult programming.  Clients in these programs are able to access an array of services such as therapy, substance abuse treatment, case management, and nursing.  The nurse in the program will be responsible for providing case management services to consumers receiving field-based intensive outpatient services.  The nurse will also assess co-morbid  issues and treat accordingly.  The nurse will work closely with the prescribing psychiatrist and and act as a liaison between psychiatry and team. 

Key Responsibilities

Work as an active team member and closely collaborate with team members.
Provide field based services as required by program.
Interface with multidisciplinary team in treatment planning and service delivery.
Maintain documentation in compliance with agency, HIPAA, and DMH standards.
Ensure that all clinical documentation is completed in a timely and accurate manner.  
Meet expected performance standards as assigned by Supervisor.

Case Manager Duties

  • Provide case management services to clients as assigned.
  • Serve as care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
  • Coordinate pharmacy services.
  • Deliver medications to consumers when necessary.
  • Facilitate medication education and health-oriented groups with consumers.
  • Provide at least 50% of services in the field.
  • Provide medication education consultations and collaboration with staff and student interns.
  • Provide basic nursing services as needed.
  • Carry a case management caseload as assigned.

Education and Knowledge, Skills and Abilities

• Some experience providing case management services required
• Must complete HIPAA training
• Must be able to pass background/criminal check.
• Valid CA Driver License and automobile insurance.
• Bachelor’s Degree preferred
• Licensed vocational nurse in California.
• Experience serving seriously and persistently mentally ill clients with co-occurring challenges. Knowledge of trauma preferred.
