Case Manager, Maternal-Infant Health

Job Overview

The Maternal-Infant Case Manager of the Partnership for Families (PFF) program will provide strength-based home visitation services with high-risk pregnant mothers or parents of an infant 0-12 months old. This is a DCFS funded child abuse prevention program. The Case Manager will provide outreach and engagement, community-based support services, home visitation, intensive case management, prenatal/postpartum health workshops, and parenting classes.   

Key Responsibilities

  • Conduct outreach and engagement, recruitment, and enrolling of participants. 
  • Conduct needs assessment, identify strengths, and collaboratively develop a case plan to help participants define goals.
  • Engage participants in ongoing follow-up and motivation to support their goals.
  • Provide prenatal and postpartum home visitation and support in areas such as breastfeeding, parent-infant attachment, child development, home safety and other maternal and infant health related topics. 
  • Provide intense Case Management including assistance with housing, benefits, employment, education, transportation, childcare, medical and other services.
  • Empathetic support and feedback when working with new moms that is trauma informed and culturally competent.
  • Connect participants to pre-natal and postpartum health services and resources.
  • Support the monthly pre-natal and post-partum health education workshops on a variety of related topics.
  • Co-facilitate and lead evidence-based parenting groups.
  • Provide advocacy with DCFS for those with an open case.
  • Maintain confidentiality in compliance with agency, HIPAA and DCFS standards.

Education and Knowledge, Skills and Abilities

  • Bachelors Degree in early child development, public health, psychology, social work or other related field.
  • Minimum 2 years work experience in maternal child health, early child development, home visitation, and/or serving families with complex needs.  
  • Preference given to those with knowledge of lactation counseling, baby sleep conditioning, and SIDS prevention. 
  • Bilingual/bicultural (English/Spanish) candidates preferred
  • Willingness to travel to provide field-based services
  • Dependable automobile, California Driver's License, and valid car insurance.
  • Must be able to pass background/criminal check.

Compensation: $36,000.00-$38,000.00
