Case Manager - Sister's Program

Job Overview

Services and interventions delivered by HealthRIGHT 360 address criminal addictive thinking, relapse prevention, successful re-entry into society, stress reduction, anger management, domestic violence prevention, self-esteem building, trauma counseling, written case management, reduced recidivism, and promotion of public safety in a correctional setting. The programs also emphasize the benefits and importance of continuing care after release from custody, and will work to link participants to treatment and supportive service programs in each county's broad network of service providers in preparation for re-entry.

Our women's programs foster personal well-being, accountability, independence, life skills training, employability, and family and community cohesion. Our Sister's program is offered in San Francisco jails.

Key Responsibilities

We are looking for a competent, organized and present Case Manager to support our Sister's Program. Case Managers are responsible for assessing participant needs and strengths in support of securing ancillary services through an array of case management services, resources and high quality customer service. Case Managers identify needed services and maintain up-to-date documentation for each participant, including a client centered case plan aimed at targeting specific client goals. Excellent documentation and computer skills are key for this position. Case Managers should be comfortable supporting diverse communities, and must be skilled with establishing rapport with clients, excellent time management skills and the ability to meet deadlines - a must!

Education and Knowledge, Skills and Abilities

You must be able to pass county and CDC background checks, highly culturally aware/sensitive and trauma informed. BA degree, prior case management experience, working with the criminal justice population and experience with spreadsheet is highly desirable.
