Case Manager/CBT Facilitator

If so, HealthRIGHT 360 is looking to fill a CBT Facilitator position in San Francsico, CA! The STEPS (Services To Enhance Parolee Success) program offers assessment, substance use disorder treatment, case management, informational workshops, educational and vocational assistance, safe housing referrals and other essential services to parolees in San Francisco. In this position, you will gain experience from working with people with chronic histories of incarceration, homelessness and addiction, working in a collaborative cross functional team and help to make meaningful changes in the lives of many, and will have the opportunity to enhance your clinical documentation skills and and learn new EMR systems.

The ideal candidate will have the ability to maintain an active caseload providing interventions as needed, ability to maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature, and the ability to facilitate CBT and other life skills workshops classes. Key Responsibilities

Individual Treatment Responsibilities

  • Facilitates individual counseling sessions with each client on the caseload
  • Offers clinical support and counseling to assist clients in meeting their treatment goals including providing feedback, setting boundaries, and celebrating successes
  • Pro-actively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status
  • Provide evidence-based CBT programs and other life skills programs to parolees

Treatment Setting Responsibilities

  • Facilitates group counseling sessions and teaches classes on various treatment-related topics with a focus on CBT
  • Conduct individual meetings utilizing motivational interviewing techniques
  • Performs crisis intervention and communicates with treatment team as unforeseen situations arise
  • Documents client updates and incidents in the facility log daily
  • Performs periodic facility walk-through to ensure and maintain the safety and security of the facility
  • The CBT Facilitator will attend required trainings and meetings
  • The CBT Facilitator Plans and facilitates client celebrations and special events
  • May work weekends and holidays as needed

Documentation Responsibilities

  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client
  • Evaluate progress of parolees
  • Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner
  • Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements

Education and Knowledge, Skills and Abilities

Required

  • The CBT Facilitator shall possess an Associates of Arts or Associates of Sciences degree from a granting institution accredited by the WASC or equivalent, have a minimum of two (2) years of experience providing CBT services to the criminal justice population; or shall possess a high school diploma, or its equivalent, and a minimum of four (4) years of experience providing CBT services to the criminal justice population
  • Registration with Drug and Alcohol Certification recognized by DHCS (within 30days of employment)
  • At least 3 years experience working in a treatment/outpatient environment with high administrative demands
  • First Aid Certified (within 30days of employment)
  • CPR Certified (within 30days of employment)
  • A valid California driver's license

Background Clearance

Required

  • Must Pass background investigation and criminal records check to obtain clearance
  • Must be able to satisfy requirements for security clearances
  • Must not be on active parole or probation 

Knowledge

Required

  • Clear understanding of appropriate staff/client boundaries
  • Culturally competent and able to work with a diverse population
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency
  • Proficiency with Microsoft Office applications, specifically Word Outlook and Internet applications

Desired

  • Knowledge of co-occurring disorders and trauma informed treatment
  • Knowledge of Clinical documentation (treatment plans, progress notes etc.)
  • Experience working with criminal justice population
  • Drug and Alcohol Certification recognized by DHCS
  • Bachelor's Degree in related field
