Certified AOD Care Coordinator

Job Overview

AOD Counselor/Care Coordinator is responsible for assessing Saint Francis Memorial Hospital (SFMH) or emergency department patients’ needs using SBIRT (screening, brief intervention, and referral for treatment) technique. Based on the assessment using SBIRT they may provide brief intervention or coordinate referral of the patient to the appropriate level of care in the community including both behavioral health and primary care services. The AOD Counselor/Care Coordinator will report to the Transition Care Manager and serve as a liaison between SFMH and the referral program/agency to ensure a seamless transition of care.

Key Responsibilities

Assessment and Patient Responsibilities: Using SBIRT (Screening, Brief intervention, and Referral for treatment technique) assesses patient for substance use disorder (SUD) using validated screening tools. Provides brief substance disorder counseling and intervention as appropriate. 

Case management/Care Coordination Responsibilities: Provides consultation and direction for patients from SFMH to ensure that behavioral health and primary medical care needs are met.  

Administrative Responsibilities:  Provide feedback to the Transition Care Manager or other assignee, to help shape the future of the Program.  Actively suggest process improvements to streamline and improve services.   Identify and determine eligibility needs based on income and eligibility criteria.   Ensure the privacy and security of PHI (Protected Health Information) as outlined in HR360’s policies and procedures relating to HIPAA compliance. 

Education and Knowledge, Skills and Abilities

Required:
 - Drug and Alcohol Certification recognized by DHCS 
 - High School diploma or equivalent 
 -First Aid Certified within 30 days of employment 
 -CPR Certified within 30 days of employment
-A valid California driver’s license 

 Desired:
 -Bachelor’s Degree in related field 

 Background Clearance 
 Required:
 -Must not be on active parole or probation  

 Knowledge
 Required:
-Culturally competent and able to work with a diverse population
-Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
-Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency
