If so, HealthRIGHT 360 is currently looking to fill a Facility Monitor position opening with our Community Prisoner Mother Program at our Prototypes Women’s Center in Pomona, CA! The ideal candidate has experience in substance abuse counseling education, prior experience at a residential treatment facility, and is familiar with the criminal justice population. In this position you will gain experience with treatment planning and documentation, working with a broad range of cultures, and working with incarcerated women. Sound interesting? Prototypes Women's center is a 180 bed residential facility for women and women with children. We provide comprehensive substance use disorder, mental health, and psychiatric care by an interdisciplinary team of professionals. Treatment episode ranges in time from 60 days to 12 months. Prototypes is a fast-based environment. The Community Prisoner Mother Program (CPMP) is a 24-bed, in-custody program located at the Pomona Residential campus. The clients in CPMP are serving a state prison sentence, housed in the community with their child(ren).

 

Key Responsibilities

  • Provide comprehensive assessments of service needs.
  • Recommend interventions to client and/or team as appropriate.
  • Facilitate individual and group interventions (e.g. skill building, education, etc.) as needed.
  • Foster and develop relationships with client family and friends.
  • Coordinate services with other involved services providers.
  • Provide referrals and linkages to services specific to client’s needs.
  • Provide follow-up to ensure services are obtained.
  • Provide crisis intervention as needed within scope of practice.
  • Communicate effectively with inter-disciplinary team and participate in team meetings to review cases.
  • Document relevant and required information regarding each client.
  • Maintain and observe all HIPAA regulations that apply.

Education and Knowledge, Skills and Abilities

  • AA Degree Preferred but not required.
  • State Substance Abuse certification or registration
  • At least two years of experience working with clients in a Mental Health, Substance Abuse, Domestic Violence and/or related field.
  • Bilingual language skill preferred, English and Spanish
  • Good written and verbal skills.
  • Strong Computer skills.
