Family Advocate Case Manager - SMC Serenity House

We are temporarily offering a signing bonus for this position of $1,000.

Join a dynamic, recovery-focused team supporting adults in San Mateo County who are in a mental health crisis. The Serenity House program offers daily recovery groups, providing skills which support recovery back to baseline functioning, establishing and maintaining healthy relationships, and promoting overall wellness. In addition to groups, the program offers one-on-one case management and psychotherapy services with the goal of serving our clients wherever they are on their recovery journeys. 

The Family Advocate has a primary role on the interdisciplinary team for engaging and involving residents and their families in their treatment while in Serenity House.

The Family Advocate will be a community professional with lived experience navigating mental health treatment/addiction services and other social services systems on behalf of the family. The Family Advocate should demonstrate skills in supporting and mentoring the families of resident clients and be able to work within a multi-disciplinary team dedicated to providing culturally sensitive, trauma-informed, and family friendly engagement practices. The Family Advocate will work closely with resident clients and their concerned significant others/family members to increase involvement, set goals, build new protective factors, and help them achieve their goals. They will also work closely with County services and other referral sources.

 

Key Responsibilities

  • Provide outreach and individualized support to residents and their families/concerned significant others.
  • Assist resident clients and their loved ones to identify strengths, needs, and priorities.
  • Advocate on behalf of loved ones and resident clients to protect their rights under the law, and link them to services and benefits to which they are entitled.
  • Educate families about the mental health/medical system and facilitate access to services and resources.
  • Participate in community outreach and training activities in coordination with other team members.
  • Other duties as assigned by the Program Director/Manager.

 

Education and Knowledge, Skills and Abilities

 Education and Experience Required:

  • Bachelor’s Degree required.
  • Able to maintain confidentiality.
  • Health screening, negative TB test, and fingerprint clearance.

 Background Clearance Required:

  • Must pass a fingerprint background check with the Department of Justice and FBI.
  • Must not be on active parole or probation, clear with OIG database.

Knowledge Required:

  • Culturally competent and able to work with a diverse population.

Skills and Abilities Required:

  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.

Desired:

  • Bilingual.

Position is Sunday, Monday, and Wednesday, 3PM - 11:30PM.

In compliance with the California Department of Public Health's mandate, all employees must be able to provide proof of COVID-19 vaccination. Medical and religious exemptions are available.

 

Compensation: $19.23
