Job Overview

Road 2 Resilience is a strength-based home visitation program for pregnant and postpartum mothers in the Pomona area. We are a child abuse prevention program focusing on families who are at high -risk for entering the child protective services system due to mental illness and/or addiction. Our goal is to surround vulnerable families with supportive services that are sustainable and grounded in the community.  

The Family Navigator will provide strength-based home visitation services with high-risk pregnant mothers or parents of an infant 0-24 months old. This is a child abuse prevention program focusing at-risk parents due to addiction or mental illness. Through our collaboration with Pomona Valley Hospital, the Family Navigator will provide client engagement, home visitation, intensive case management, prenatal/postpartum workshops, and parenting classes.   

Key Responsibilities

  1. Conduct outreach and engagement of pregnant or postpartum mothers at Pomona Valley Hospital, local OBGYN Clinics, or the community
  2. Conduct needs assessment, identify strengths, and collaboratively develop an Individualized Service Plan to help participants define goals.
  3. Provide prenatal and postpartum home-visitation and support in areas such as breastfeeding, parent-infant attachment, child development, home safety and other maternal and infant health related topics. 
  4. Provide case management services including assistance with housing, benefits, employment, education, transportation, childcare, medical and other services.
  5. Engage participants in ongoing follow-up and motivation to support their goals.
  6. Empathetic support and feedback when working with new moms that is trauma informed and culturally competent.
  7. Avert crises by assisting the family in times of urgent need (risk of harm to child or caregiver, pending child removal), in consultation with the Clinical Home Visitor and Supervisor
  8. Connect participants to pre-natal and postpartum health services and resources.
  9. Co-facilitate meetings and engage community leaders in our coalition.
  10. Co-facilitate and lead evidence-based parenting groups.
  11. Responsible for maintaining all clinical documentation at a high standard, completed in a timely manner, and accurate.   
  12. Collect all pre and post surveys and outcome measures.
  13. Maintain confidentiality in compliance with agency, HIPAA and DCFS standards
  14. Comply with all agency’s policies, procedures, and codes of ethics.
  15. Attend internal and outside meetings as assigned.

And, other duties as assigned by manager.

Education and Knowledge, Skills and Abilities

1.     Required: Bachelor’s Degree: from an accredited program in the field of psychology, erly child development, or related field;

2.     OR Certified Alcohol and Drug Counselor (CADC) or Certified Advanced Alcohol and Drug Counselor (CAADC).

3.     Minimum 3 years field-based clinical work experience with at-risk families who have complex needs.  

4.     Bilingual English/Spanish is desirable.

5.     Experience with evidence-based, culturally competent, gender responsive, and trauma informed services.

6.     Willingness to travel to provide field-based services

7.     Dependable automobile, California Driver's License, and valid car insurance.

8.     Must be able to pass background/criminal check.

Background Clearance

Required: Live scan DOJ and FBI fingerprint clearance;

-Must not be on parole or probation for a minimum of two years


Required: Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications

Skills and Abilities

Required: Ability to provide high level customer service to a client population struggling with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
