Job Overview

The Healthright360 California Community Reinvestment Grant, will provide services to client's within Contra Costa county in the areas of Mental Health Treatment and Substance Use Disorder Treatment. Healthright 360 in partnership with Contra Costa county manages the Reentry Network. The Network works with AB 109 and functions as a broad-based collaborative system working in partnership with local law enforcement, county agencies, community based and faith based organizations to provide integrated whole-person care and reentry services designed to optimize successful outcomes and reduce recidivism.

The Contra Costa California Community Reinvestment Grants Program and the Re-entry Network substance use disorder counselor/Field Operations Coordinator is responsible for providing substance use treatment services to clients who are under the supervision of the Contra Costa County AB109 Probation Department and providing linkages to community based resources. Including housing, education, employment, family, recovery, mental health, etc. Counselor will adhere to the curriculum specified in the grant for those clients referred to the CCCRG program, an internal program for those receiving mental health treatment, substance use disorder treatment, and system navigation services in Contra Costa County.

Key Responsibilities

Individual Treatment Responsibilities:

  • Maintains a case load of 20-30 clients.
  • Facilitates group counseling sessions and teaches classes according to specified curriculum topics specified in the grant.
  • Facilitates individual counseling sessions with each client on case load.
  • Will offer clinical support and counseling to assist clients in meeting their treatment goals including providing feedback, setting boundaries, system navigation services in Contra Costa County and celebrating successes.
  • Conduct and participate in case conferences for each client assigned to caseload.
  • Attend required trainings and meeting.
  • Maintains appropriate boundaries to ensure the adherence to the identified safety and security of a criminal justice setting.
  • Collaborates with Re-Entry Network case managers in all areas of client care.
  • Proactively links clients (those disproportionately impacted by past federal and state drug policies) to both internal and external resources based on their treatment needs for and follows up on the progress/status.

Documentation Responsibilities:

  • Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in a timely manner.
  • Works with clients and other available internal and external resources to develop/maintain treatment plans, progress notes and appropriate updates in support of health and recovery needs of the client.
  • Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT360 to satisfy internal and external evaluating requirements.

Education and Knowledge, Skills and Abilities

  • The Certified SUD counselor or registered counselor shall possess and Associate of Arts or Associate of Science degree from a granting institution or equivalent and/or high school diploma or equivalent; and a minimum of three (3) years full time experience of counseling duties with a criminal justice populations in a substance abuse program; and have a Certified SUD Counselor Certification or be registered by an agency approved and recognized by the California Department of Health Care Services (DHCS).
  • A valid California driver’s license.
  • Experience engaging and working with reentry population.
  • Experience with case management and Knowledge of community resources.
  • Knowledge and understanding of issues and dynamics within the formerly incarcerated community relating to reentry and reintegration.
  • Culturally competent and able to work with a diverse population from various backgrounds.
  • Excellent public and community relations, negotiation, mediation, conflict management, and meeting facilitation skills.
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook and Internet applications.
  • Experience working successfully with issues of substance abuse, mental health, criminal background and other potential barriers to economic self-sufficiency.

Compensation: $60,000.00-$63,000.00
