If so, HealthRIGHT 360 is currently looking to fill a Homeless Outreach Coordinator position opening at our Perinatal Outpatient - Serenity TOO program, located in Oceanside, CA. The ideal candidate has experience with outreach and networking, Gender Responsive, Trauma Informed services, and the Integrity to handle sensitive information in a confidential manner. In this position, you will gain experience working with women and children as well as additional experience working with Substance Abuse, Mental Health, Criminal Justice and Child Welfare Services populations. Perinatal Outpatient, Serenity TOO, provides treatment services for women seeking recovery from substance use disorders. We offer gender responsive, trauma informed, person-centered services and evidence based interventions. For clients with co-occurring mental illness, we provide integrated substance use and mental health services which treat both conditions as primary.


Key Responsibilities

  • Link participants to primary care, dental care, vision care, hearing care, mental health, addiction and recovery, food, clothing, housing, homeless prevention, communication, transportation, employment, SSI, SSD, Medical, CAPP and financial services, among other things.
  • Collaborate with Program Director and treatment team to prioritize participants and their need of services.
  •  Research sources for needed resources and establishes rapport with shelters and other programs in the area serving the homeless.
  • Travel to community sites where case management services may have been reduced and service needs are unmet.
  • Assists program participants in accessing shelters, food pantry services and assisting with completing housing applications for subsidized housing opportunities. Assists participants in housing search, obtaining and maintaining their housing. 
  • Provide case management services for homeless individuals as assigned by Program Director, by assessing needs, developing and monitoring case plans, connecting individuals with community resources and advocating for participants.
  • Collaborate with agencies and volunteers to create partnerships that cultivate resources and stay informed about any recourse changes.
  • Demonstrated ability to prioritize multiple important tasks and meet deadlines in a fast-paced environment.

Education and Knowledge, Skills and Abilities

• Some experience providing therapy & case management services for children and families
• Must complete HIPAA training
• Must be able to pass background/criminal check.
• Valid CA Driver License and  Valid CA Automobile Insurance.
• Bachelor’s Degree preferred
• Preferred Knowledge Includes: Trauma-Informed care, 0-5 years old, Nonviolent Parenting & Education
• Spanish speaking preferred
