Housing Services Coordinator

Job Overview

A multi-sector partnership of the County, the city of Antioch, and HealthRIGHT 360, CoCo LEAD Plus will provide pre-arrest, pre-booking, and post-booking diversion opportunities and services for adults with behavioral health issues who have been arrested by the Antioch Police Department (“APD”) more than one in twelve months for eligible misdemeanors or “wobblers.” Probationers arrested on divertible charged in Antioch or cited for technical violations will also be eligible.

Key Responsibilities

The Housing Services Coordinator is responsible for:

  • Act as Primary Point of Contact (PPOC) to all housing organizations that provide housing services to CoCo LEAD Plus and its clients, including SLEs, transitional houses, and project-funded master-leased housing provider
  • Work as part of a multi-disciplinary team to assess, place and track clients in housing
  • Develop and maintain relationships with local housing providers that house or could potentially house CoCo LEAD Plus clients
  • Conduct field visits to housing sites where CoCo LEAD Plus clients reside and ensure that the clients and providers are compliant with program and funding requirements
  • Provide daily support to clients to ensure they are able to maintain housing and engagement in other project services
  • Address and work to resolve any housing-related crises and other events as they arise

Education and Knowledge, Skills and Abilities

Desired attributes, skills and experience:

  • Experience assisting clients with completing housing applications, leases, and other housing-related documentation
  • Experience conducting the VI-SPDAT homelessness assessment
  • Experience in peer engagement, support or mentoring services
  • Substantial knowledge of the local community (regions, neighborhoods, community resources)
  • Substantial knowledge of Contra Costa County housing resources
  • Knowledge of or willingness to train in techniques such as harm reduction, motivational interviewing, stages of change, cognitive behavioral treatment, restorative justice, gender responsivity, trauma-informed care, de-escalation and safety, client confidentiality, and documentation
  • Attention to detail and information management
  • Strong oral and written communication skills
  • Engaging and supportive personality
  • Proficient in Microsoft Office applications (Excel, Outlook and Word) and internet applications
  • Experience working with complex populations


  • High school diploma or equivalent
  • Valid California Driver’s License, reliable vehicle, and willingness to travel regularly (mileage allowance provided)
  • Must undergo a background investigation and criminal records check; ability to be approved for clearance into juvenile and adult custodial facilities
