Clinical Care: Assist clinicians in the examination process. Room all patients for Addiction medicine clinic providers, taking vital signs and assisting patients in competing any questionnaires, GAD7, CIWA, COWS deemed necessary by the Addiction medicine providers. Perform office tests including vision screening, EKG, UA and urine pregnancy tests. Perform phlebotomy, injections, and TB tests. Administer Vivitrol injection and manage the panel of patients receiving the medication. Working with Addiction Medicine Team to fit patients into the schedule. Actively participates in addiction medicine team quality improvement and quality assurance activities.
Preparation Responsibilities: Huddle with the addiction medicine team or specific providers as directed. Perform intake, including vital signs, list chief complaints, allergies, immunization histories and complete chart forms, as appropriate to patient type. Place patients in rooms and prepare them for examination. Clean, prepare and stock examination rooms.
Coordination Responsibilities: Active participation in integrated team based care which may include receive and return telephone call from patients and pharmacies. Assist in scheduling patient appointments. Monitor and ensure quality assurance of basic clinic functions. Oversee patient flow and ensure smooth, efficient patient flow throughout patient’s clinic experience via patient centered care and playing an active role in the team dance, which may include but is not limited to: chart prep, huddles, red carpeting the patient, 30 second report, mid-way check in, discharge of the patient. Assist the AMOA in panel management of patients being treated with Buprenorphine/SBX
Gather medical information pertaining to addiction from patients visiting ERs or being hospitalized.
Serving as a liaison daily with information sharing with the primary care, mental health and behavioral health team.
