Mental Health Case Manager II - Serenity House

Job Overview

The Mental Health Care Coordinator II is responsible for assessing client needs and strengths in support of health and recovery. Through a gender responsive, trauma informed approach, the Mental Health Care Coordinator II provides counseling in individual and group settings using curriculum in accordance with program specific requirements under the supervision of a licensed clinical professional. Additionally, the Mental Health Care Coordinator II provides individual case management, care coordination and referrals to assist clients in navigating systems of care while providing a supportive treatment environment.  

Key Responsibilities

Utilizing a gender responsive, trauma informed approach; the Mental Health Care Coordinator II supports in designing and implementing the clinical group/activity program at Serenity House; facilitates individual counseling and group counseling as needed; Participates in treatment and team meetings as necessary; Collaborates with Serenity House staff and community providers in order to complete Treatment Plans and Transition Planning. In addition, they provide crisis management interventions as needed and handle all necessary follow ups.

 

Individual Treatment Responsibilities: Facilitates individual counseling sessions with each caseload client; Provides clinical support to assist clients in meeting their treatment goals. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status; Assists Therapists in conducting appropriate mental health assessments and completing treatment plans.

 

Treatment Setting Responsibilities: Facilitates group counseling sessions and community meetings and provides psycho-educational groups on various treatment related topics. Performs crisis interventions and communicates/coordinates with the treatment team as unforeseen situations arise.   As needed, accompanies clients to offsite appointments related to their treatment. Attends required trainings and meetings.

 

Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client. Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements. Develops treatment plans, completes release and consent forms, etc. In addition, completes Medicare and Medi-Cal documentation and billing as needed.

Education and Knowledge, Skills and Abilities

Education and Experience

Required:

-Masters Degree in related field (Psychology, Counseling, Social Work, etc.)

-Experience working with people in extreme states; or person’s experiencing psychosis, or suicidality, or other forms of profound distress

-First Aid Certified within 30 days of employment

-CPR Certified within 30 days of employment

 

Desired (any of the following):

-Direct experience working client’s in extreme states

-Direct experience working with high-intensity environments

-Lived experience of either being diagnosed, or systematized, or hospitalized

-Training in deeper listening skills, such as reflective listening and Motivational Interviewing

-Demonstrable ability to convey accurate empathy

 

Background Clearance

Required:

-Must pass a fingerprint background check with the Department of Justice and FBI

-Must not be on active parole or probation, clear with OIG database

 

Knowledge

Required:

-Culturally competent and able to work with a diverse population

-Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications

-Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency

 

Desired:

-Knowledge of DBT, CBT, MI and trauma based treatment highly desirable

-Experience navigating 5150 or emergency/crisis procedures

Desired:

-Bilingual

-Knowledge of trauma, co-occurring disorders and trauma-informed treatment

-Knowledge of Clinical documentation (treatment plans, progress notes etc.)

-Experience working with criminal justice population

-Experience working with crisis prevention and de-escalation techniques
