Mental Health/Behavioral Health Therapist - ICC

Job Overview

In addition to collaborating with each client’s primary care provider, the therapist will assess clients and provide them treatment primarily by utilizing short-term Evidence Based Practice (EBP) guidelines. The program is predominately used with clients who exhibit mild to moderate symptoms of mental health disorders such as depression, anxiety, PTSD and substance abuse in adults. Prior experience with EBPs is desirable. The therapist will be a member of the Behavioral Health team and reports to the Behavioral Health Manager.

Key Responsibilities

  • Provide mental health screenings and initial assessments to clients referred through the Integrated Care Center (ICC).
  • Assist in the assessment, diagnosing, planning, arranging, and implementing of client treatment plans.
  • Provide short‐term, structured, evidence‐based psychotherapy as part of an appropriate treatment plan and in accordance with HR360 contracts.
  • Coordinate and collaborate with the primary care providers, and other referring providers, to improve health outcomes.
  • Facilitate referrals to other services (e.g. case management, substance abuse treatment, specialty care and community resources) as needed.
  • Provide clinical therapeutic services to clients, including individual and group therapy and coordination of treatment for participants with co‐occurring disorders.
  • Provide brief intervention and crisis intervention as needed, including collaboration with other departments to provide behavioral health/mental health services and support.
  • Work to meet client needs effectively and resolve individual barriers through follow‐up, advocacy and collaboration with HR360 staff and other community service providers.
  • Maintain accurate and timely records of activities, progress notes, and services provided to each client.
  • Participate in and/or facilitate team meetings.
  • Prepare and utilize psychosocial educational materials as appropriate, needed or required.
  • Use tests, measurements, and measurement scale for general and specific assessments in order to attain objective client criteria in the provision of services and evaluation procedures.
  • Provide in-service presentations to staff as needed or required.
  • Provide regular, written reports to the Behavioral Health Manager and/or the Director of Medical Services and additional reports as assigned.
  • Other duties as assigned to support the mission of the organization.

Education and Knowledge, Skills and Abilities

  • Familiarity and comfort working with issues related to homelessness, substance abuse, mental health diagnosis, and history of trauma.
  • Spanish-speaking preferred but not required.

Experience in the following areas:

  • Psychosocial assessment
  • Individual therapy
  • Group therapy
  • Short‐term, evidence‐based counseling
  • Crisis intervention

Background Clearance Required:

  • Must not be on active parole or probation.

Knowledge Required:

  • Culturally competent and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.
