Occupation Therapist - Part-Time

Job Overview

Road 2 Resilience is a strength-based home visitation program for pregnant and postpartum mothers in the Pomona area. We are a child abuse prevention program focusing on families who are at high -risk for entering the child protective services system due to mental illness and/or addiction. Our goal is to surround vulnerable families with supportive services that are sustainable and grounded in the community.  

The Occupation Therapist will provide strength-based home visitation services with high-risk pregnant mothers or parents of an infant 0-24 months old. This is a child abuse prevention program focusing at-risk parents due to addiction or mental illness. The OT will provide home-based sessions on baby bonding, emotional and sensory regulation, and helping babies meet developmental milestones.

Key Responsibilities

  1. Provide rehabilitative services to prenatal and postpartum parents and caregivers.
  2. Assist client in developing independent living skills to promote independence and self-sufficiency, as developmentally and clinically appropriate.
  3. Assist client in developing regulation strategies, positive lifestyle, and positive parenting strategies.
  4. Co-facilitate and lead psycho-educational, socialization, rehabilitation, and life-skills groups.
  5. Provide empathetic support and feedback that is strength-based, trauma informed, and culturally competent.
  6. Conduct outreach and engagement of pregnant or postpartum mothers at Pomona Valley Hospital, local OBGYN Clinics, or the community.
  7. Work as an active member of a multi-disciplinary team in the provision of family-focused services.
  8. Serve as care coordinator for clients who are cross referred to Regional Center Services. 
  9. Develop a collaborative client-centered Case Plan and update client progress.
  10. Work within OT scope of practice, as part of treatment team in collaboration with the clinical home visitor, family navigator, physician, therapist, and other team members.
  11. Provide crisis assistance, as needed, within scope of practice
  12. Co-facilitate meetings and engage community leaders in our coalition
  13. Responsible for maintaining all clinical documentation at a high standard, completed in a timely manner, and accurate.
  14. Collect all pre and post surveys and outcome measures
  15. Maintain confidentiality in compliance with agency, HIPAA and DCFS standards
  16. Comply with all agency’s policies, procedures, and codes of ethics.
  17. Attend internal and outside meetings as assigned.

And, other duties as assigned by manager.

Education and Knowledge, Skills and Abilities

Education and Experience


  • MA or MS from regionally accredited Occupation Therapy graduate program. OR currently pursuing advanced Occupation Therapy degree
  • NBCOT Registration and CBOT License expected within 6 months of hire. 
  • Valid CA driver’s license and current automobile insurance. 
  • Spanish/English speaking ability preferred.

Background Clearance


-Live scan DOJ and FBI fingerprint clearance

-Must not be on parole or probation for a minimum of two years



-Culturally competent and able to work with a diverse population

-Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications 

Skills and Abilities


-Ability to provide high level customer service to a client population struggling with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
