Job Overview

Prototypes Mental Health Center in Pasadena provides comprehensive mental health services for men, women and children. Services range from prevention and early intervention, to more intensive services for those with serious and chronic conditions. Staff provide services at Prototypes and in the community. These can include individual and group therapy, medication management and medication-assisted therapies, case management, and referrals and coordination with primary care clinics.

Responsible for providing case management services to consumers receiving field-based intensive outpatient services. Support and assist consumers in their recovery. Utilize medical expertise to provide services and support as part of a treatment team.  Provide medication management education. Conduct groups on medication management. Carry a case management caseload. Chart as needed.

Key Responsibilities

General Responsibilities:

  • Assist medication providers to meet the psychiatric needs of the clinic’s adult, TAY, and child populations. 
  • Work as an active team member and closely collaborate with team members.
  • Provide field-based services as required by program.
  • Interface with multidisciplinary team in treatment planning and service delivery. 
  • Develop and assess effectiveness of individualized treatment plans and consumer progress.
  • Maintain timely documentation in compliance with agency, HIPAA, DMH, and other relevant regulatory body standards.
  • Perform other duties as assigned by Supervisor.
  • Arrange work schedule in accordance with the agency’s needs.
  • Comply with the agency’s policies and procedures.
  • Attend internal and outside meetings as assigned.
  • Meet expected service hour goals as assigned by Supervisor.
  • Coordinate pharmacy services.
  • Deliver medications to consumers when necessary.
  • Facilitate medication education and health-oriented groups with consumers as assigned.
  • Provide medication education consultations and collaboration with staff and student interns.
  • Provide basic nursing services as needed.
  • Assist in crisis and symptom management.

Documentation Responsibilities:

  • Submit program and contractual reporting and documentation requirement in a timely manner. 
  • Monitor and document for medical quality assurance.
  • When providing direct services, complete all required paperwork, charting, records maintenance in a timely manner.

Education and Knowledge, Skills and Abilities

  • Licensed vocational nurse in California. 
  • Experience serving severe and persistently mentally ill clients with co-occurring challenges. 
  • Knowledge of trauma preferred.
  • At least two years of experience working with clients in a Mental Health Environment
  • Bilingual language skill preferred, English and the predominant other language natural to the community the facility is located  
  • Good written and verbal skills
  • Dependable automobile and insurance, registration and valid California Drivers License
  • Knowledge and respect of all confidentiality issues

  • Bilingual in fluent Spanish (read, write, speak)
