Parent Partner/Case Manager

Job Overview

The Family Wellness Center at Prototypes, Pasadena supports mental health wellness for children and families of our community through providing a menu of services, including case management, psychotherapy, occupational therapy, groups for children, and medication support services.  Services are rendered in the field, school, home, and in the office, based on the meeting the needs of the family and child. 

The Children's Case Manager is responsible for providing resource linkage to clients/families in their communities, as well as supporting skill development in order to enhance mental health of the child and family. The ideal candidate has experience working as a part of a collaborative team within a DMH setting and possesses excellent time management and documentation skills. Additionally, this candidate will be knowledgeable about children and families, as well as the systems that surround them. Ideal candidates must have "lived experience" as a caregiver of a child who is currently receiving or who previously received services from the public mental health system. Spanish speaking language ability is required. 

Key Responsibilities

  • Must have lived experience in caring for a child who is currently receiving or who previously received services from the public mental health system. 
  • Some experience providing case management services to children and family required.
  • Provide case management services to clients/families, including providing services where the client lives and/or attends school,  assisting with housing, benefits, employment, education, transportation, child care, medical and other needed services, as developmentally appropriate.
  • Provide referral/linkage and parenting services to parents/caregivers of assigned clients. 
  • Serve as care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
  • Assist client in developing coping skills to promote achievement of developmentally appropriate milestones. 
  • Co-facilitate and lead psycho-social rehabilitation groups.
  • Timely completion of DMH documentation and strong organizational skills. 
  • Assist in crisis and symptom management.
  • Office and field-based services.

Education and Knowledge, Skills and Abilities

  • Some experience providing case management services for children and families.
  • Must complete HIPAA training.
  • Must be able to pass background/criminal check.
  • Valid CA Driver License and  Valid CA Automobile Insurance.
  • Bachelor’s Degree preferred.
  • Preferred Knowledge Includes: Trauma-Informed care, 0-5 years old, Parenting with Non-violence training.
  • Spanish speaking required.


Compensation: $21.00
Type: Hourly/ Non-exempt
