Patient Safety Navigator - Intake (On-Call)

Job Overview

Are you familiar with various supportive counseling strategies? Do you have knowledge of the behavioral health and public health systems? If so, HealthRight 360 is looking to fill a On-Call Patient Safety Navigator position in San Francisco.

The Patient Safety Navigator (PSN) will work within the Integrated Care Center and support client navigation to services within HealhRight 360 and the broader community.

The PSN will be responsible for welcoming and assisting patients, and helping maintain a safe environment for clients, staff, and visitors. Provides excellent customer service and addressing any immediate needs of patients. 

Key Responsibilities

  • Welcomes and assists patients with navigating the clinical resources throughout the Integrated Care Center.
  • Ensures and maintains a safe and orderly environment for clients, staff, and visitors.
  • Make regular rounds to monitor entrances, exits, offices, common areas, and building perimeter.
  • Assists with incident reporting, surveillance video retrieval, and grievances.
  • Work collaboratively with service providers and administrative staff to maintain an orderly environment.

Education and Knowledge, Skills and Abilities

  • HS Diploma or GED required.
  • Ability to interface well with diverse populations.
  • Ability to respond quickly and use good judgment in unusual and emergency situations.
  • Excellent verbal and written communication, planning and time management skills.
  • Proficient in MS Word, Excel, PowerPoint, and Internet applications.
  • Good attention to detail.
Compensation: TBD
Type: Hourly/ Non-exempt
