PRCS Case Manager

Job Overview

PRCS CM are responsible for working in conjunction with Subcontracted Community Based Transitional, Sober Living to assess participant needs and strengths in support of developing and securing permanent, independent or supportive housing and ancillary resources.

Key Responsibilities

Reviews participant information with CBP and collaboratively coordinates placement of participants directly into post transitional housing and employment resources.  Monitors and tracks CBP services delivery for QA on contracted community programs.  Responsible for documenting participant status and progress for permanent housing Maintain accurate records through entering data/documentation into electronic database for all participants in accordance with guidelines established by Healthright360 to satisfy internal and external evaluating requirements.

  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data

  • Strong initiative and enthusiasm and willingness to pitch in whenever needed
  • Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations
  • Able to work within a frequently changing project scope while maintaining overall direction and structured priorities
  •  High School diploma or equivalent
  •  A valid California driver’s license
  •  Ability to clear and maintain a background clearance (must be off of Parole or Probation)
  •  Culturally competent and able to work with a diverse population
  •  Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
  •  Experience working successfully with issues of criminal background, and other potential barriers to economic self sufficiency.
