At our Integrated Care Center, primary care providers work with a team of nurses, medical assistants, case managers, mental health providers, dentists, and other specialists to deliver care and improve health outcomes for underserved populations.

Key Responsibilities

- Part-time position
- Provides comprehensive care, within the scope of credentialed privileges and standardized procedures, to  patients within a Patient Centered Medical/Health Home delivery model 
- Works collaboratively with mental health, addiction, and dental staff to facilitate integration between primary care,  addiction, mental health, and oral health in order to improve patient outcomes
- provides on-call after-hours availability on a rotating basis, as requested by supervisor;
- continuing professional education programs (up to 5 days covered per year)
- Provides clinical leadership to support staff, including nurses, medical assistants, patient care coordinators, front office staff, receptionists, and interns/students
- Demonstrates comfort in changing systems, champions change, and leads change processes.
- Meets benchmarks for applicable quality indicators as noted in the annual quality management plan including the development of an appropriately sized continuity panel of patients and the maintenance of current productivity standards; 
- Assists in continuous quality improvement through on-going problem/needs identification and problem solving
- Supervises students, interns, and residents as appropriate

Education and Knowledge, Skills and Abilities

- Current license in the state of California
- X-waiver (must obtain upon hire)
- Board Certification or Eligibility, Internal Medicine or Family Practice if applicable
