Job Overview

This position will be based at our Tenderloin Health Services clinic which is located in the historic Glide building in the Tenderloin neighborhood of San Francisco.  It is a vibrant, fast-paced, dynamic clinic that seeks to provide care in non-traditional models and is determined to meet clients "where they are at" by employing harm-reduction methods and continuous judgment-free unconditional care.

A Registered Nurse renders professional nursing care to patients, within an assigned unit or authorized HealthRIGHT360 (HR360) medical clinic site. The RN may work on a patient care team to provide nursing support. Provides patient care under standing orders, health education, and advanced patient care support. Ensures integrated healthcare team support of patient encounters, medical case management and quality improvement. Provides complex care management for patients with multiple chronic medical illnesses and behavioral health concerns. Supports integrated healthcare team to ensure efficient clinic flow. In addition, this position will perform duties assigned on an as needed basis.

Key Responsibilities

Nursing Responsibilities: 

  • Manages “same day” patients with primary care team. 
  • Provides advice to primary care team walk-in patients and patients calling into clinic. 
  • Provides nursing support to the patient care team and managing clinic flow.
  • Provides appropriate patient care visits and manages patient panel under standing orders.
  • Manages prescription refills and medication modification for patients under standing orders.
  • Interviews patients for health history, including history of current complaint, and past and present use of medication utilizing patient identifies; document patient charts
  • Initiates follow-up, as necessary. Instruct patients on healthcare issues, as indicated by physician, diagnosis and/or inquiries by patient.
  • Submits correspondence/documentation for patient referrals within care coordination system to ensure proper patient referrals.
  • Assess emergency situations, critical lab values, telephone encounters and urgent condition changes and immediately report them to the appropriate Provider.
  • Order vaccines and lab tests for patients, and collect, handle, and process lab specimens appropriately and according to policies and procedures.
  • Support and enforce infection control policies, practices and procedures.; promote a work environment of service, teamwork and respect
  • Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency.
  • Work courteously and respectfully with other employees, follow direction and work well under pressure.

Organizational Responsibilities: 

  • Maintain standards of accurate and complete client record keeping and reporting.
  • Responsible for properly and consistently documenting required information accurately on Clinic records and reports. 
  • Keeps up to date with operational and procedural requirements.

And, other duties as assigned.

Education and Knowledge, Skills and Abilities

Education and Experience

Required:

·         BA or equivalent degree.

·         Graduate of an accredited School of Nursing,  Active California RN license,

·         BLS Certification (must obtain upon hire)

 

 

Desired:

·         2 years or equivalent experience

·         Bi-lingual in Spanish

 

Background Clearance

Required:

-Must not be on active parole or probation 

 

Knowledge

Required:

-Culturally competent and able to work with a diverse populations

-Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications

-Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency

- Proficient in language in addition to English

- Experience in managing volunteers

 

Preferred:

- Experience with electronic medical records.

- Comfort working on teams and with safety net populations.

Desired:

-Knowledge of co-occurring disorders and trauma informed treatment

-Knowledge of Clinical documentation (treatment plans, progress notes etc.)

-Experience working with safety net and criminal justice populations
