HealthRIGHT 360 is currently looking to fill a Registered Nurse position opening at our Cordilleras Mental Health Rehabilitation Center in Redwood City, CA! The ideal candidate has two years of direct patient care experience working in a primary care setting, experience working with diverse populations including those who are mentally ill, and has successfully worked with issues of substance abuse, mental health, and medical complexity. In this position you will have the opportunity to creatively design and implement performance improvement projects that ensure that all residents are receiving needed preventive and chronic care.  Additionally, this position offers the ability to work independently and as part of team, utilizing the full scope of your RN license in an integrated behavioral health/primary care model! 

Cordilleras Mental Health Rehabilitation Center (MHRC) houses two separate treatment programs for adults with chronic mental illness; a licensed 68-bed mental health rehabilitation center (MHRC) and a licensed 49-bed adult residential facility (ARF) called The Suites. Cordilleras primarily serves San Mateo County residents, 18 and older, with long histories of mental illness and multiple episodes of acute psychiatric hospitalization. The goal of Cordilleras is to allow individuals who would otherwise be in a state hospital or an acute care setting to develop the skills and supports needed to live more independently in the community. Cordilleras also offers treatment options for persons who have substance abuse issues in addition to a serious mental illness.  

Telecare provides care for the patients while they are on the unit, incorporating such approaches as motivational interviewing, cognitive behavioral therapy, dialectical behavioral therapy (DBT), harm reduction, and other consumer-centered therapeutic interventions in addition to medication management by a psychiatrist.

HealthRIGHT 360 will be taking over primary care services for the residents of the rehabilitation center in a separate clinic on the first floor.  In addition, we will be working closely with Telecare to ensure that all residents, whether or not they are seen in the primary care clinic, are receiving appropriate preventive care and management of their chronic illnesses.

Key Responsibilities

  • Provide nursing care, within the scope of license and agency credentials, to residents of the Cordilleras MHRC seen within the primary care clinic on site, utilizing trauma-informed, motivational interviewing skills and encouraging patient activation and self-management.
  • Facilitate team functioning and efficiency by arranging the schedule of patients to be seen with the Telecare team and taking responsibility for rooming, vitals, etc. to ensure the completion of patient sessions in a timely and thorough manner.
  • Take responsibility for patients with complex medical issues, facilitating referrals and communicating plans with the Telecare team.
  • Work collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes.
  • Review lab results and ensures appropriate follow-up of abnormal results through the medical provider and/or Telecare as appropriate.
  • Follow-up with patients and the Telecare team regarding recent medical emergency room visits and medical inpatient stays.
  • Lead continuous quality improvement efforts, ensuring appropriate chronic and preventive care for all patients residing on the unit, whether or not they have been seen by the medical team.

Education and Knowledge, Skills and Abilities

  • Possession of a valid RN license
  • Bilingual/fluent in English/Spanish preferred
  • Two years of direct patient care experience in a primary care setting or a community health center, preferred
  • Experience working successfully with issues of mental illness, substance abuse and criminal backgrounds
  • Knowledge of co-occurring disorders and trauma-informed care
  • Excellent verbal, written and interpersonal skills
  • Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility
  • Strong initiative and enthusiasm and willingness to pitch in whenever needed
  • Experience working with electronic health records (E-clinical works, preferred)


Salary is commensurate with experience.  HealthRIGHT 360 offers full medical benefits.
Hours are 5 days a week Monday – Friday for 20 hours (part-time)
