SUD Counselor II/III - Prototypes CPMP

Job Overview

Prototypes Community Prisoner Mother Program is an in-custody, long term residential program located on the campus of Prototypes Women's Center in Pomona. Prototypes is a 180 bed facility providing substance abuse and mental health services to women and women with children.

The SUD Counselor II/III is a registered substance use disorder professional who maintains registration from an approved/accredited California agency. The SUD Counselor II/III is responsible for delivering services to a caseload of clients and providing individualized substance use disorder services. The SUD Counselor II/III works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission and vision. The SUD Counselor II/III may provide treatment utilizing different modalities including individual and/or group. Services may be provided in: outpatient office, residential treatment, or the field, based on client and program need.

Key Responsibilities

  • Assists clients with intake, assessment, and orientation to program upon admission or transition in care level.
  • Helps lead the client through the treatment plan and delivers individual and group substance use disorder services.
  • Monitors client's progress toward achieving treatment plan goals and provides treatment plan input. 
  • Utilizes trauma informed care while applying motivational interviewing, cognitive behavioral therapy, and evidence-based practices.
  • Delivers residential and intensive outpatient service clients with a minimum of 1 individual session per week.
  • Delivers outpatient clients a minimum of 1 session every two weeks. 
  • Seeks clinical consultation throughout the treatment episode and when encountering new and/or high risk clinical circumstance. 
  • Works collaboratively with other members of the treatment team including the Case Manager, Clinical Therapist, Medical, and Psychiatrist.
  • Maintains frequent communication with the treatment team and engages in regular consults, clinical team, case conferences, and plan development meetings.
  • Delivers services in in-custody, residential, or field depending on assignments and client need.
  • Responsible for treatment oversight of the clients on their caseload.
  • Assists with client program oversight by managing client business passes, privileges, check-in, and treatment compliance. 

Education and Knowledge, Skills and Abilities

  • Registered Substance Abuse Counselor with a State of California approved agency.
  • Basic understanding of child development, child abuse, substance abuse  and self sufficiency issues. 
  • Bilingual English/Spanish is preferred.
  • California Driver's License with proof of insurance.
