If so, HealthRIGHT 360 is looking to fill a full-time TAY Case Manager position in San Francisco, CA! We are looking for a bilingual, Spanish-speaking case manager to work with transitional age youth (18 - 24 years old) in jail. Our goal is to stop the trauma to prison pipeline for young people who are incarcerated. This position is primarily assessment, using the Adverse Childhood Experiences (ACEs) Tool to screen for trauma. Some case management and linkage is involved. A knowledge of San Francisco resources and jail populations is necessary. Good clinical documentation skills and the ability to work well as part of a team are necessary!

Key Responsibilities

  • Conducts trauma screenings and behavioral health evaluations with individuals incarcerated in the San Francisco City and County Jail who are between the ages of 18 and 24
  • Collaborates with patients to develop comprehensive reentry plans
  • Provides evidence-based and trauma informed care to all patients in both individual and group treatment settings
  • Makes referrals to community treatment providers as clinically indicated
  • Works with existing treatment providers, family members and friends to prevent the patient’s disconnection from their natural community supports
  • Acts as a liaison between the patient’s natural community supports and the criminal justice and legal systems
  • Works closely with San Francisco Superior Court’s Collaborative Court programs including Young Adult Court, Drug Court and Behavioral Health Court
  • Routinely assesses for client safety
  • Charts on patients
  • Develops treatment plans
  • Refers to psychiatrist for medication when indicated
  • Follows all policies and procedures of the program

Education and Knowledge, Skills and Abilities

  • Bachelors in Social Work, Psychology or similar field required, Masters preferred
  • Bilingual Spanish and English preferred
  • Ability to work in a jail setting
  • Organized, works well independently and as part of a team
  • Prefer experience with ACEs Tool and with young adults
