If so, HealthRIGHT 360 is currently looking to fill a Care Coordinator III position opening at our Asian American Recovery Services-Adult Outpatient program in San Francisco, CA. The ideal candidate has experience working with substance use and mental health issues, diverse populations and the criminal justice system and and understanding of facilitating groups. AARS Project ADAPT is an outpatient treatment program that provides integrated substance use and mental health services to adults who reside in San Francisco .  We believe that behavioral health is a fundamental part of overall wellness for all families and communities. Key Responsibilities

Individual Treatment Responsibilities

  • Conducts comprehensive client needs assessment and diagnosis, monitors and facilitates client progress
  • Facilitates individual counseling sessions with each caseload client
  • Carries a client caseload and provides mental health and substance abuse treatment and meets productivity goals
  • Assists clients in developing individualized service plans with specific goals, objectives, and measurable outcomes to address mental health and substance abuse issues
  • Make referrals and provide linkages to relevant services and treatment sources
  • Provide crisis intervention and case management services as needed

Treatment Setting Responsibilities

  • Facilitates and conducts group counseling sessions
  • Attends weekly supervision, clinical case conferences, department, agency wide and training as determined appropriate by supervisor
  • Serves in a multi-disciplinary team and ensures appropriate level of care is provided to the client
  • Develop and implement curriculum as needed to meet the changing client needs in the program
  • Utilizes community resources and programs as available

Documentation Responsibilities

  • Monitors and documents client progress in accurate and timely manner in the electronic data systems
  • Assists in developing and maintaining of confidential client charts/information
  • Maintains quality assurance standards and performs regular audits for compliance and accuracy of data
  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the mental health and substance abuse needs of the client
  • Properly documents all individual and group counseling sessions and completes the discharge paperwork/process and required agency assessments in timely manner
  • Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360
  • And, other duties as assigned

Education and Knowledge, Skills and Abilities


  • Master’s Degree in related field
  • BBS registered as LCSW, LMFT, LPCC or ASW/AMFT/APCC
  • At least 2 years experience in mental health and substance abuse/treatment programs
  • Experience running group counseling sessions
  • Knowledge of clinical documentation (treatment plans, progress notes etc.)
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook and internet applications
  • First Aid Certified within 30 days of employment
  • CPR Certified within 30 days of employment
  • Valid TB test clearance


  • Experience working with API and diverse communities
  • Working experience with evidence based strategies (CBT, DBT, Seeking Safety, Motivational Interviewing)
  • Knowledge of co-occurring disorders and trauma informed treatment
  • Experience working with criminal justice population

Background Clearance

  • Must not be on active parole or probation
