Job Overview

HealthRIGHT 360 is currently looking to fill a Therapist/LPHA position opening at our Engaging Positive Action Youth & TAY Program in East Palo Alto, CA. The ideal candidate has experience working with people with histories of homelessness, substance use and co-occurring disorders. This individual has good clinical documentation skills and loves working as a part of a team. In this position you will gain experience working in a collaborative cross functional team and help to make meaningful changes in the lives of many. 

Engaging Positive Action Outpatient is a trauma-informed program aimed to serve youth (12-17 y/o) and transitional age youth (18-25 y/o) who are struggling with substance use disorder and co-occurring mental health issues. We strive to meet the ongoing needs of the individual with empathy, compassion and respect.

Utilizing a gender responsive, trauma informed approach, therapists have clinical responsibilities and LPHA/assessment responsibilities. This position will have the ability to carry a caseload of individual clients in in the outpatient substance use disorder program and managed care mental health. As a therapist, this person will provide individual and group therapy/counseling, collaborate with other providers both internal and external to HR360 to assist clients in completing their treatment plan goals and participate in discharge planning. In addition this individual will provide crisis management as needed and handle all necessary follow-up. As LPHA, this person will administer intake and admission assessments, create comprehensive treatment plans, sign off on treatment plans and other drug required drug Medical documentation.

Key Responsibilities

Individual Treatment Responsibilities

  • Provides weekly individual therapy and counseling to clients in the outpatient treatment facility, and to their partners and families when clinically appropriate
  • Maintains confidentiality while working as part of a treatment team and adhering with program norms
  • Participates in case conferences, treatment planning and clinical discussions to provide holistic care
  • Communicates with prescribing psychiatrists regarding psychotropic medication compliance, side effects, and follow up appointments

Treatment Setting Responsibilities

  • Facilitates evidence-based psycho-education groups on a variety of core curriculum topics including CBT, Seeking Safety, DBT, Grief and Loss, etc.
  • Coordinates discharge planning with client’s Care Coordinator
  • Provides consultation to staff regarding symptom management, mental health conditions and crisis intervention strategies
  • Attends individual and group supervision, attends staff and agency meetings
  • Participates in agency trainings as required
  • Complies with Board requirements for license registration/renewal
  • Provide individual therapy to clients in treatment as needed, and to their partners and families when clinically appropriate
  • Communicate with prescribing psychiatrists regarding psychotropic medication compliance, side effects, follow-up appointments
  • Coordinate discharge planning in collaboration with the rest of the treatment team
  • Provide consultation to staff regarding symptom management, mental health conditions and crisis intervention strategies
  • Comply with Board requirements for license registration/renewal
  • Participate in the development of treatment plans and assessments and complete all assigned documentation in a timely manner

Documentation Responsibilities

  • Daily maintenance of monitoring of EMR report, to prevent and clear any compliance errors in records
  • Conducts assessments, develops treatment plans, completes release and consent forms, and other required documentation in various electronic systems
  • In addition, completes Managed-care, MediCal and Drug MediCal documentation and billing requirements
  • Documents individual and group services delivered in various electronic systems in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements

And other duties as assigned.

Education and Knowledge, Skills and Abilities

Required

  • Licensed or Board Registered MFT or MFTi, LCSW or MSW
  • Minimum two years of experience in dual diagnosis substance abuse treatment in an inpatient or outpatient community mental health setting
  • Ability to obtain and maintain background clearance with DOJ and FBI
  • Culturally competent and able to work with a diverse population
  • Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data
  • Professionalism, punctuality, flexibility and reliability are imperative

Desired

  • Bilingual
  • Knowledge of DBT, CBT, MI and trauma based treatment highly desirable
  • Knowledge of trauma, co-occurring disorders and substance abuse treatment
  • Knowledge of Clinical documentation (treatment plans, progress notes etc.)
  • Experience working with criminal justice population
