Transitional Counselor - Folsom Men's Program

Job Overview

The Transitional Counselor is responsible for providing support to participants in transitions to continuing care upon parole.  This is achieved by providing guidance to participants and to advocate for participants to access continuing care services in their home communities.  In conjunction with appropriate specialized and/or CDCR staff, the Transitional Counselor works with participants to develop a continuing care plan for participants prior to client parole date.  The Transitions Counselor serves as liaison between the in-custody program and the Specialized Treatment for Optimized Programming (STOP) network.   

Key Responsibilities

Treatment Planning Responsibilities: Provides information and guidance to the participants on available outpatient and residential treatment programs and any other services developed by CDCR or California Countries. Assist participants in completing the appropriate forms regarding the transitional care phase of the program services. Work in cooperation the other contract staff, CDCR parole staff, the continuing care service provider county probation, and the participant for the development of a community service plan and discharge plan. In conjunction with clinical assessment and treatment plan review teams, the participant, and institutional parole department, evaluates participant’s continuing care needs and develops community services plan 180 days prior to parole.

Treatment Setting Responsibilities: Attends and facilitates specialized groups, prepares lesson plans, teaches classes, workshops, and seminars, plans and executes clinical days related to continuing care.  Participates in development of continuing care-related curriculum materials. Works in collaboration with prison staff by attending joint meetings, required trainings and upholding all norms, policies, and procedures of prison. 

Administration and Compliance Responsibilities: In accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external administrative demands, those of the CDCR/prison, and other evaluating agencies, maintains accurate records on all participants and files reports as required. Helps develop informational program for community providers in order to familiarize them with needs of SUD parolees. Documents participant attendance in continuing care presentations and meetings with parole, CCIII and/or the PSA to discuss plans. 

Education and Knowledge, Skills and Abilities

Education and Experience


- Ability to work with diverse population

- Ability to express empathy and compassion and communicate honestly by providing feedback for negative as well as positive behaviors


Must fulfill one of the following:

  • Associates Degree in Human Services/Social Sciences and two (2) years of experience providing CBT to the offender population; 


  •  High School Diploma or its equivalent and four (4) years of experience as a certified counselor providing CBT to the offender population.


- Specialized knowledge such as, mental health issues, life skills, vocational training, anger management, incest, criminal behavior, rape crisis, grief, etc.

- Knowledge of the TC milieu, 12-step philosophy, or other approaches to recovery.

- A second language (particularly Spanish)

- Computer literacy

- Familiarity with penal codes and the criminal justice system


Background Clearance

- Must not be on active parole or probation 

- Ability to receive and maintain CDCR clearance



- Culturally competent and able to work with a diverse population

- Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications

- Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency

- Knowledge of co-occurring disorders and trauma informed treatment

- Knowledge of Clinical documentation (treatment plans, progress notes etc.)

- Experience working with criminal justice population

- Bilingual
