As a member of the housing supportive services team, under the direct supervision of the Housing Supervisor, the Bridge Intensive Case Manager will support clients enrolled in Housing for Health Bridge Interim Housing program. This program serves individuals who have experienced long-term homelessness, history of trauma, and have long-standing health and mental health issues.
