The Community Outreach Worker works in collaboration and continuous partnership with the healthcare team, to provide community outreach in order to link individuals to the HealthRIGHT 360 Medical clinic for integrated primary care, behavioral health, substance use disorder treatment, oral healthcare, enabling resources and other services provided at the health center.
