Function: Substance Use Disorder

The Care Coordinator is responsible for providing substance abuse prevention and early intervention services to a diverse client population. The Care Coordinator will provide community based outreach services, pre-treatment case management services including individual counseling sessions. The Care Coordinator will also provide client referrals to primary care, substance use and co-occurring treatment programs based on screening and assessments. The Care Coordinator works directly with clients under the general supervision of the Program Manager.

MAPS provides mentoring and support to individuals who are justice-involved, participating in one of the San Francisco Superior Court's Collaborative Treatment Courts program and struggling with one or several of the following: mental illness, a substance use disorder, and/or experience in the armed forces.

Haight Ashbury Free Clinic (HAFC) at the Integrated Care Center (ICC) offers primary medical, dental, substance use disorder treatment, and mental health services guided by the principle that "Healthcare is a Right, Not a Privilege." 

HAFC/ICC provides compassionate, nonjudgmental care to patients who experience homelessness, poverty, and addiction. Our providers have specialized expertise in preventative health, acute and chronic conditions, the treatment of infectious disease, and addiction.
