Certified Community Behavioral Health Clinic (CCBHC)

Certified Community Behavioral Health Clinic Expansion- integrated Outpatient behavioral (mental health and substance use disorders treatment) and physical health program funded by SAMHSA’s Certified Community Behavioral Health Expansion (CCBHC-E) grant project; now funded through 09/30/2026 with a secondary round of funding secured: Improvement & Advancement (CCBHC-IA). Project EACH- Enhancing Access to Comprehensive Healthcare: goal is to integrate primary care with a local Federally Qualified Health Center (FQHC) into our behavioral health (mental health and substance use disorder) services, allowing clients to receive a whole-person, streamlined system of care, targeting their primary care and behavioral health needs.

The CCBHC program enhances the Outpatient Behavioral Health programming for clients aged 21+ through our Prototypes, Pomona location.

Hours:
Monday- Friday 8:00 am- 5:00 pm

Eligibility for services:
Clients who enroll in behavioral health treatment programming (Substance Use and/or Mental Health) are simultaneously enrolled in CCBHC which enhances the services and resources available.

To enroll in a behavioral health treatment program, potential clients can contact the Prototypes, Pomona Access Call Center by calling (909) 634-2950.

Sliding Scale fees for Private Pay Program
Fees are to be paid in full before services are rendered. Fees maybe paid in cash or money order. For court mandated clients: If a letter or certificate required– additional $25 administrative fee is due.
All incomes are gross, before taxes.
