Our Pomona campus includes a 32-unit affordable housing complex, the S. Mark Taper Foundation Family Living Center, for low-income, single-parent families. We provide case management and counseling services to all families living within the complex. Many families transition from the residential program to the Family Living Center.
This aftercare plan is critical to ensure the continued recovery of clients and support for their families. Case managers provide assessment and treatment planning, link clients to important resources in the community, monitor progress and advocate for the client. They are critical in supporting a client’s decision-making and help address barriers to treatment or continued recovery.
