Integrated Care Center
Integrated Care Center provides compassionate, non-judgmental care. Our supportive staff is experienced with the complex healthcare needs that can result from homelessness, poverty, and addiction. Our providers have specialized expertise in preventative health, chronic conditions, the treatment of infectious diseases, and addiction.
Services
- COVID vaccine
- Evaluation and treatment of medical issues
- Integrated primary care and behavioral health services
- Dental services for adults (children on Saturdays)
- Follow-up of chronic and ongoing medical problems
- Routine checkups
- Screening for many health disorders and chronic disease management, such as diabetes (high blood sugar), high blood pressure, high cholesterol, anemia, hepatitis, and cancers
- Women's health care services, including family planning, pregnancy test, complete obstetrical services, breast exams, mammogram referrals, and cervical cancer screenings
- Geriatric health care services
- Behavioral health services (individual and group therapy)
- Psychiatric Services
- Confidential HIV and Hepatitis C testing, counseling, and treatment
- Substance use services, including Medication Assisted Treatment (MAT)
- Tobacco cessation treatment
- TB screenings and treatment
- In-house phlebotomy
- Food Pharmacy
- Case management, housing resources, and employment services
- On-site charter school to earn a high school diploma or GED
- Acupuncture
- Nutrition counseling
Integrated Care Center Clinic Hours:
Monday through Friday, 8:45 AM – 12 PM and 1 PM – 4:45 PM
Closed for lunch 12:00 PM—1:00 PM
