Enhanced Care Management (ECM)

HealthRIGHT 360 (HR360) is contracted with DHCS Medi-Cal Managed Care Plans (MCP) in various counties across California to provide Enhanced Care Management services. Enhanced Care Management (ECM) is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of members through systematic coordination of services. ECM is comprehensive care management that is community-based, high-touch, and person-centered. The goal is to help people engage in care, stay engaged in care, and take control of their health.

Members are assigned to a Lead Care Manager (LCM) that meets them wherever they are — on the street, in a shelter, in their doctor’s office or at home — to meet their needs. LCMs provide assessment and screenings; linkages and navigation to primary care; BH services; social supports; appointment management and support; comprehensive transitional care; comprehensive care planning; member and family supports; and benefit enrollment services to ECM members.

Monday – Friday, hours vary slightly by program

Eligibility for services

HR360’s target population for ECM services include adults who: are transitioning from incarceration, experiencing homelessness, are at risk for avoidable hospital or emergency department utilization, and/or with serious mental illness (SMI) and/or substance use disorder (SUD) needs. Interested participants must be assigned to contracted Managed Care Plans and meet DHCS additional criteria. Please call to inquire about eligibility.
